Transformation of Services for Children with Disabilities (CwD)
Programme

What is it about?
Worcestershire County Council is currently looking to change the
way services for children with disabilities are delivered. As part
of this programme we are looking at five areas:
Redesign of Short Breaks Provision
Our aim is to make sure that there is the right
balance of types of short breaks for referred children and young
people, enabling their families to have breaks from caring
responsibilities. The range of services include overnight stays,
school-based, voluntary and community sector provision.
Community
Short Breaks Commissioning Strategy (PDF 182 KB). Find out

Choice and Control (Personalisation)
Our aim is to give families more choice and control over how
they spend their allocated money on services for their children
with disabilities and complex health needs.
Integrated Health and Social Care Teams
Our aim is to create single teams with all the health and social
care skills that families need.
Specialist Equipment
Our aim is to create a single equipment resource that will allow
professionals, and the families they work with, to access equipment
more readily in order to meet the changing needs of children and
young people within their homes.
Transitions
Our aim is to ensure that children and young people receive the
support they need as they move into adulthood.
Why are we doing this?
We want to make sure Worcestershire offers the best quality
services and meets the needs of families by:
- Delivering improved health and social care outcomes
- Delivering services which are sustainable, effective and
of a high quality
- Reducing the costs to achieve improved outcomes and
efficiency.
The government is also asking local
authorities to make sure that every child has a shared assessment
for health, social care and education and families have a say and
choice.
Who will it affect?
This project will include everybody involved with children with
disabilities.
Worcestershire County Council's Joint Commissioning Unit is
consulting on the Worcestershire Short Breaks Statement that has
been reviewed to make clearer the range of available short break
activities and how these can be accessed. This is to enable carers
of children with disabilities to have breaks from their caring
responsibilities. The review was done in accordance with the
Breaks for Carers of Disabled Children Regulations 2011.
