The single assessment process, introduced in the NSF for older people, aims to make sure older people's care needs are
assessed thoroughly and accurately, but without procedures being needlessly duplicated by different agencies.

agencies across Worcestershire are working to share and reduce duplication of clinical and care information and where practical pre-populate information fields.
Training is available to clinical and care staff to promote the beneifts and take up of the single assessment process. Worcestershire
has a Single Assessment tool that has 3 parts; ‘Contact’ Assessment that gathers generic data about the individual ‘Consent’
Form which is appropriately completed. ‘Overview’ Assessment that gathers a chronological history of events in specific
areas of an individual’s life. Copies of each of these documents are attached under the assessments pack page As
a way of co-ordinating the completed documentation (and providing an appropriate multi-disciplinary folder) within an individual’s home the ‘Red’ Single Assessment
and Care Record are available. |
